Treatment Agreement for the Sex Offender Treatment Program

This document outlines the professional requirements and commitments necessary to participate in this specialized therapy program. This program is designed for individuals who are serious about addressing and changing their problematic sexual behaviors. Your participation signifies a commitment to this process and a willingness to engage in the necessary work for personal growth.

Client Commitments:

Honesty and Transparency: I agree to be open and honest with my therapist about my past and present sexual thoughts, fantasies, and behaviors.
Behavioral Change: I am committed to changing unhealthy and problematic sexual behaviors, as evidenced by the following:
- Abstinence from pornography.
- Cessation of all deviant or unhealthy sexual behaviors.
- Limiting sexual contact and fantasies to the context of an equal, honest, committed, consensual, and appropriate relationship.
Substance Use: I will not use illegal drugs or alcohol during treatment. I understand and agree to random drug testing at my therapist's discretion and accept responsibility for any associated costs.
Digital Monitoring:
- I will not use any unmonitored cell phones or other electronic devices.
- Upon acceptance into the program, I will install an approved internet filtering software, Accountable2You, on all my electronic devices (phones, tablets, laptops, and desktops). The use of any other software requires prior approval from my therapist.
Assessments: I agree to participate in any recommended risk or psychological assessments. I understand that I am responsible for any additional costs associated with these evaluations.
Social Media: I will not access any social media networking sites or apps, including but not limited to Facebook, Snapchat, Twitter, and Instagram.
Residential and Contact Restrictions:
- I will not share a bedroom or any other sleeping accommodations with any child under the age of 12.
- I will not have overnights with anyone, including family members. If an overnight stay with a family member is necessary, they must have no children in the home and be fully aware of my behavioral issues.
- I will not have unsupervised contact with any minor child under the age of 16. Any contact with children must be supervised by an adult at all times, including during bathroom visits.
- If attending a school activity in which I am a participant, I must be accompanied by a supervising adult who is aware of my behavioral issues at all times. This includes during trips to the bathroom.
